The UDNF is a non-profit organization supporting the Undiagnosed Diseases Network (UDN) and people with undiagnosed and ultra-rare diseases. Funded by the NIH, the UDN research study looks to solve the most challenging medical mysteries. The UDNF’s Patient Navigation program supports UDN applicants, participants, and the larger community with undiagnosed and ultra-rare conditions.

Is the UDNF Patient Navigation Program Right for You?
How the Patient Navigation Program Can Help You:
Provide information and answer questions about the UDN application process and criteria.
Assist with applying to the UDN.
If accepted to the UDN, collaborate with you and your UDN site to support your participation in the program.
Provide timely updates and answers to questions between the UDN site staff and patients.
If you are not a participant of the UDN, help with guiding you to resources, navigating the healthcare system, and additional opportunities you may qualify for.
To qualify for the UDNF Patient Navigator Program, you, or your child or dependent, must meet one of the following qualifications:
Interested in applying to the Undiagnosed Diseases Network (UDN).
Have applied to the UDN.
Be a participant in the UDN.
Applied but was not accepted to the UDN.
Have an ultra-rare disease diagnosis with no dedicated advocacy or community group for your ultra-rare disease. An ultra-rare disease is defined as affecting less than 1 in 50,000.
Have an undiagnosed disease, defined as having had multiple tests and seeing multiple specialties with no diagnosis result.
Must live in the US or be in the US military living abroad.
What the Patient Navigation Program Does Not Do
UDNF Patient Navigator Program does not provide medical services or advice.
We do not provide case management outside the UDN.
UDNF Patient Navigators do not make decisions on UDN acceptance.
